The student insurance provider will be United Healthcare.
Voluntary Domestic Students: Visit to enroll in the student health insurance. Payment will be made directly to United Healthcare. Details on the plan below.
Graduate Assistants:
The monthly cost for a Graduate Assistant will be $129 and it will be deducted from stipends received September through May. Graduate Assistants must waive the coverage if they currently have another health insurance policy.
Please visit to opt in the coverage or waive it. Remember as a GA, you will not pay the insurance company directly for your individual coverage. To waive the coverage, you will have to provide United Healthcare information on your current insurance through the above.
The last day to waive is September 13th. If a waiver has not been completed by September 13th, you will be automatically enrolled in the policy. No refunds or waivers will be approved after the deadline.
Students must have a referral from Moffitt Health Center before the insurance will pay. No referral is required if medical care is received when the student is more than 50 miles from campus.

Although the federal government pays a portion of the costs, Medicaid is administered and operated by states, and each state's program is a little different depending on the needs and goals of that state. The student health insurance plan coverage begins on August 1st and ends on July 31st each year. It is the responsibility of each Graduate Assistant to review options and make decisions about their health insurance coverage. In order to waive the University鈥檚 student plan you must have proof of other coverage to provide during the August Enrollment Period each year.
