Adding a form through the university-approved provider called Formstack is a quick and easy process that will only require a few steps. If you need help getting started creating a form in Formstack, iTech has uploaded the for everyone to refer to. You can also submit a work order through iTech to get started using Formstack or if you have other issues with creating forms.
Embedding a form in a Omni CMS page
Login to Formstack and select the form, you want to embed. Copy the script code from your Formstack form located on the Share tab of the form admin page. It should look something like this:

Next, create an asset in Omni CMS by going to Content --> Assets --> New. Select the Source Code option in the overlay box. Name your form using the template "Forms - {Name of your form}. Copy your script into the Asset Content section. It should look like this:
Publish the asset.
Now navigate to the edit tab of page that you would like your form to be included on and select the Insert Asset button. There you can choose your form asset. It should look like the below in the edit screen:
After saving and publishing, your form should show up on your page like below:
